About: CHI Living Communities is looking for a Director of Community Connections to join their team. The role involves fostering partnerships and ensuring seamless transitions across their Continuum of Care Connections Program, including Skilled Nursing, Independent Living, Assisted Living, Memory Care, Home Care, and Community-based Services.
Requirements: Bachelor’s degree in Healthcare Administration, Social Work, or a related field; Experience in Long-Term Care, Case Management, Home Care, Hospice, or Transitional Care preferred (related experience considered); Strong communication and relationship-building skills
