Position: Community Health Worker – Health Information Assistance

Requirements: High school diploma or GED and completion of an online 30-hour training program and certification exam required; previous experience in a health care or community setting; experience in related program and volunteer work preferred, or an equivalent combination of education and experience sufficient to successfully perform the essential duties of the job listed; CHW Level I pathway opportunity to certification, with the expectation that all CHWs will be certified by 12/31/2025 aligned with current MDHHS CHW bulletin
