Position: Care Concierge

Requirements: High school diploma or GED required; Bachelor’s degree strongly preferred; Certification as a Community Health Worker preferred; experience working with vulnerable populations, substance use, and/or mental health disorders for at least 2 years preferred; experience in a community-based setting for 1 to 2 years preferred; demonstrated knowledge of community-based services in Philadelphia; familiarity with West Philadelphia a plus; experience working as a member of an interdisciplinary team; experience navigating complex healthcare and social services systems
