Position: Traveler Nurse Practitioner or Physician Assistant – Assisted Living and Home Settings – OH, OR, WA, WI
About: Optum Home & Community Care, part of the UnitedHealth Group family of businesses, is seeking a dedicated individual for a Senior Community Care (SCC) program role that involves providing care to senior patients at home, assisted living facilities, and independent senior living facilities. This position offers a unique opportunity to contribute to an integrated care model that addresses the physical, mental, and social needs of patients across various care settings.
Requirements: Active NP or PA license or ability to obtain by start date; Active ANCC or AANP national certification for NPs; Current, active DEA licensure or eligibility to obtain post-hire; 1+ years of clinical or geriatric practice experience; Willingness to travel and obtain additional licensure in assigned areas; Proficiency with electronic medical records
