Position: MDS Coordinator Nurse
About: Evergreen Healthcare Group comprises a network of 11 skilled nursing and post-acute care centers across California and Washington, dedicated to providing quality care to residents and peace of mind to their families. Village Health Care in Gresham, Oregon, is part of this network and is currently seeking a dedicated MDS Coordinator Nurse to join their team.
Requirements: Must possess a Clear & Active LVN/LPN or RN license in the state of employment; DSD Certified preferred; Minimum two (2) years of experience in a post-acute/skilled nursing facility, LTC, or hospital; Must have experience in supervision and principles, practice of teaching, etc.
