Position: LPN – Field Nurse | Home Healthcare
About: ChristianaCare HomeHealth, a part of Christiana Care Health System, is seeking a dedicated and meticulous LPN to join their team in New Castle, DE and surrounding areas. The successful candidate will work day shift hours with one weekend a month, 2 holidays a year, and two on-call shifts a month, providing skilled nursing care and treatment to geriatric patients in the community.
Requirements: Current license to practice as an LPN in the State of Delaware; Associates degree or completed an accredited LPN education program; Work experience in an acute or chronic care facility, or home health agency is preferred; Must be IV proficient; Develop and implement care plans; Develop discharge plans for patients; Proof of a valid driver’s license and active auto insurance is required
