Position: IRIS Consultant
About: TMG is seeking a compassionate and genuine individual with a background in social services or human services to join our team. This remote position involves partnering with individuals enrolled in the Wisconsin IRIS Program and the TMG IRIS Consultant Agency, offering support and guidance to help them maximize the benefits of the Medicaid long-term care option. ICs play a crucial role in assisting participants from diverse backgrounds in achieving their long-term care goals and living independently. The position requires close collaboration with clients to develop personalized IRIS plans and connect them with community resources.
Requirements: Bachelor’s degree in social work, psychology, human services, counseling, nursing, special education, or a related field; 1+ year of direct experience in social services for individuals with disabilities and older adults; ability to work independently with minimal supervision, strong problem-solving and time management skills, proficiency in technology use, valid driver’s license and reliable transportation
