Position: Host Home Provider – Arizona – Scottsdale and Tempe
About: Mosaic at Home is seeking Home Providers in the Scottsdale and Tempe, AZ areas to join their shared living service for individuals with disabilities. Providers open their homes to individuals with disabilities, offering support, mentorship, and assistance with daily living tasks, fostering independence and community inclusion. Duties vary based on the level of support needed, ranging from minimal assistance to full support, with income matching the level of care provided. Responsibilities include skill acquisition, medication administration, medical appointments, health support, documentation, transportation, and socialization opportunities.
Requirements: Must be a resident of the state of Arizona; own or rent a home in the state; at least 21 years of age; experience preferred but not required; reliable computer and internet access; complete all state mandated trainings; pass State/Federal and APS/CPS background check; maintain an environmentally safe and clean residence
