Position: Host Home Provider – Arizona – Scottsdale and Tempe
About: Mosaic at Home is seeking Home Providers in Scottsdale and Tempe, AZ. As a shared living service provider for individuals with disabilities, Mosaic at Home has been enhancing the quality of life, stability, and community access for nearly 40 years. Home Providers offer a safe and supportive environment while assisting individuals with daily living tasks and fostering independence through personalized care and activities both at home and in the community.
Requirements: Must be a resident of the state of Arizona; own or rent a suitable home for shared living; at least 21 years of age; experience preferred but not required; reliable computer and internet access; complete all state-mandated trainings; maintain an environmentally safe and clean residence; ability to pass State/Federal and APS/CPS background check
