Position: DIETICIAN III – ACHN – AMBULATORY
About: The Cook County Health and Hospitals System is seeking a Dietician III for the Network Diabetes Program. This role involves educating on nutrition, providing individual and group education, and conducting assessments for patients with diabetes.
Requirements: Active registration with the Commission on Dietetic Registration (Must provide proof at the time of interview); Licensed as a Dietitian by the State of Illinois (Must provide proof at the time of interview); Five (5) years of work experience in diabetes program development in a healthcare setting; Must possess active certification as a Diabetes Care & Education Specialist (CDCES); Bilingual; Knowledge of diverse cultures as related to dietary habits; Good organizational skills; Excellent oral and written communication skills; Clinical assessment skills; Excellent instructional skills; Ability to work flexible work schedule; Ability to apply learned knowledge of modern principles and practices of Medical Nutrition therapy; Ability to interact and communicate effectively with other members of the Health Care Team for the enhancement of patient care; Ability to communicate effectively with patients
