Position: COMMUNITY HEALTH WORKER, EMERGENCY DEPARTMENT-GRANT FUNDED

About: Cook County Health and Hospitals System is seeking a Community Health Worker to join their team at the Emergency Department, located at Stroger Hospital in Chicago, IL. This position is grant funded and involves providing care coordination services to patients, facilitating interactions between patients, medical services, and community-based organizations within the County of Cook.

Requirements: High school diploma or GED (Must provide proof at time of interview); One year of experience performing health risk screenings, providing health education, or supporting linkages to community-based resources; Previous experience with software and computers, e.g., word processing, appointment scheduling, Microsoft Office
