Requirements: Provides behavioral health services to patients and families who have acute chronic mental and behavioral health needs within their respective care setting; experience in mental health risk assessment, evidence-based therapy, safety planning, and medical provider consultation; ability to work effectively in a multi-disciplinary integrated care team; familiarity with chronic health condition management, trauma intervention, and behavioral health screening
