- Current state-issued physical therapist license or eligibility
- CPR certification
- Strong communication, assessment, and problem-solving skills
- Ability to work independently in home environments
- Valid driver's license

Job Duties

- Collaborate with interdisciplinary home health team members
- Educate patients and caregivers on exercises and safety
- Document visits and assessments accurately and timely
- Support patients in maximizing independence and quality of life
