Job Requirements
- 3-5 years of experience in client-facing roles preferably in senior care
- LPN certification preferred
- Experience in care management and team support
- Strong communication and interpersonal skills
- Knowledge of ADLs and IADLs
- Experience in private duty or home care services
- Organizational skills and attention to detail
- Proactive and service-oriented mindset
- Proficiency in care management software and Microsoft Office
- Physical ability to lift 50 pounds
- Valid driver’s license and transportation
Job Qualifications
- 3-5 years of experience in a high-touch, client-facing role preferably in senior care, private duty, or hospitality
- LPN certification strongly preferred
- Proven experience in care management and managing complex client relationships
- Strong communication, interpersonal, and problem-solving skills with high discretion and professionalism
- Knowledge of ADLs and IADLs for senior care clients
- Experience working in a private duty or home care service company
- Exceptional organizational skills and attention to detail
- Proactive, service-oriented mindset with passion for extraordinary care
- Proficiency in WellSky or other care management software and Microsoft Office 365
- Ability to lift up to 50 pounds
- Valid driver’s license and reliable, professional transportation
Job Duties
- Serve as the primary liaison with community partners including Activities Directors, Executive Directors, Resident Committees, and sales and marketing teams
- Collaborate with non-competing senior care providers such as physical therapy, occupational therapy, hospice, and home health partners to support comprehensive care
- Conduct in-depth, personalized consultations and assessments with clients and families to develop tailored care plans
- Guide clients and families through the intake process including agency registration, care planning, and budgeting for services
- Manage a select caseload of clients as the main point of contact overseeing all aspects of their care
- Provide coverage for client shifts on an emergency basis when caregivers are unavailable
- Support hiring decisions and assist in training and mentoring caregivers
- Match clients with caregivers based on skill set, experience, personality, and preferences
- Conduct regular quality assurance visits and check-ins to ensure care plans are followed and clients are satisfied
- Anticipate evolving client and family needs and offer proactive solutions
- Serve as a dedicated liaison between clients, families, customer service, and scheduling teams to facilitate clear communication
- Maintain accurate, thorough, and confidential records for clients and caregivers
- Work scheduled hours every other weekend to support urgent client needs
- Partner with leadership to continuously improve service offerings and maintain market leadership
- Participate in process improvement and system integration projects as a super user of cloud-based business systems
- Plan, promote, and facilitate weekly community events to enhance engagement
- Support community-planned on-site and off-site events
- Visit potential clients and families at rehab facilities or hospitals to coordinate safe transitions home and initiate care services
