Job Requirements

  • Requires BA/BS degree
  • Minimum of two years of experience working with a social work agency or equivalent
  • Strong computer skills including Excel, Outlook, and Electronic Medical Records
  • Ability to perform telephonic, virtual, and occasional in-person outreach
  • Knowledge of long-term services and supports, behavioral health, and physical health coordination
  • Must comply with company vaccination policies
  • Willingness to travel up to 10% of the time for field work
  • Must be within reasonable commuting distance to work location unless accommodation is granted
  • Ability to document and monitor care plans
  • Excellent communication and documentation skills

Job Qualifications

  • BA/BS degree or equivalent combination of education and experience
  • Minimum two years of experience working with social work agencies or related fields
  • Strong computer skills including proficiency in Excel, Outlook, and Electronic Medical Records
  • Experience with case management especially for older adults or individuals with disabilities preferred
  • Knowledge of healthcare related fields preferred
  • Ability to work independently and in virtual and field environments
  • Strong communication and interpersonal skills
  • Understanding of person-centered planning and care coordination principles

Job Duties

  • Perform telephonic or virtual outreach to individuals in specialized programs, providers, or stakeholders to support care plans and meet contractual outreach requirements
  • Conduct in-person visits as needed based on business needs
  • Submit utilization or authorization requests with documentation supporting care plans
  • Monitor Person-Centered Support Plans (PCSP) and consult with primary service coordinators when risks related to LTSS, physical or behavioral health are identified
  • Engage with healthcare management and interdisciplinary teams to provide coordinated care support
  • Manage non-clinical needs of members with chronic illnesses, co-morbidities, or disabilities for efficient utilization of long-term services
  • Document members' short- and long-term service and support goals in collaboration with care teams
  • Identify members who may benefit from alternative services or waiver programs
  • Serve as mentor, subject matter expert, or preceptor for new staff
  • Report critical incidents to appropriate internal and external parties including state and county agencies
  • Assist in appeals, fair hearings, member grievances, and state audits
