Job Requirements
- Bachelor’s degree or Master’s degree in Physical Therapy from an accredited college
- Licensed Physical Therapist issued by the state in which you will be working
- Minimum of one year healthcare experience
- Current driver’s license and ability to spend approximately 20 percent of your day driving to and from patient locations
- Commitment to consistently meet critical deadlines for charting
- Skills needed to self-manage your time and schedule
- Demonstrated experience with tablets, mobile phones and EMR software
Job Qualifications
- Bachelor’s degree or Master’s degree in Physical Therapy from an accredited college
- Licensed Physical Therapist issued by the state in which you will be working

- Demonstrated proficiency with tablets, mobile phones, and EMR software
- Strong time management and scheduling skills
- Commitment to meeting charting deadlines
- Valid driver’s license
Job Duties
- Collaborate with healthcare team members to develop and implement patient care plans
- Provide physical therapy services to patients in their homes within Caledonia and surrounding areas
- Manage patient documentation and ensure timely and accurate charting
- Utilize tablets, mobile phones, and EMR software for patient care and communication
- Maintain compliance with licensing and regulatory requirements
- Travel approximately 20 percent of work time between patient locations
- Participate in ongoing training and professional development opportunities
