Job Requirements
- Master’s degree from an accredited NP program or PA program
- Active California state Nurse Practitioner or Physician Assistant license
- Active RN license and furnishing number (NP only)
- Nurse Practitioner Board Certification or Physician Assistant Certification
- NPI number, DEA registration, valid BLS certification
- Valid California driver license and current automobile insurance
- Ability to communicate positively, professionally, and effectively with others
- Effective problem-solving, organizational, and time management skills
- Experience with older adult (geriatric) patients preferred
- Previous clinical or home care experience preferred
- Compliance with HIPAA regulations
- Comfort with end-of-life care discussions
- Ability to perform physical tasks including standing, walking, and lifting up to 20 pounds
Job Qualifications

- Effective communication skills
- Strong problem-solving, organizational, and time management abilities
- Experience caring for older adult (geriatric) patients preferred
- Previous EMR experience preferred
Job Duties
- Conduct in-home assessments with some outpatient clinical work on health plan members
- In-home assessments including comprehensive history, physical exam, medication review, and cognitive/depression/safety screenings
- Comfort with end-of-life care discussions
- Laboratory specimen collection in the home setting when appropriate
- Identify diagnoses for active medical and care management focusing on chronic disease management
- Communicate assessment findings to inform the PCP of potential gaps in care
- Provide education to patients, families, and caretakers with emphasis on monitoring and follow-up needs
- Knowledge of appropriate community resources for referral
- Comply with all HIPAA regulations and maintain security of protected health information
