- minimum of 5 years clinical experience
- bachelor's degree required
- supervisory experience preferred
- able to demonstrate professional competencies in home care setting
- ability to function as a member of an integrated healthcare team
- proficient with technology including cell phone and computer use
- able to travel within service area and HRHS entities
- current PA or NJ professional license
- valid driver's license
- CPR certified

Job Duties
- supervise clinical staff
- coordinate home care service delivery
- implement and evaluate patient care plans
- ensure compliance with government regulations and ACHC standards
- provide direct patient care as needed
- assume 24-hour accountability for patient care functions
- direct patient care team activities
