Job Requirements

  • high school diploma or equivalent
  • two years of business or related work experience
  • able to work independently demonstrating sound judgment
  • available for on-call duty outside of normal office hours
  • valid driver license and use of insured automobile
  • strong interpersonal and decision-making skills

Job Qualifications

  • two years of business experience
  • prior experience in home care industry or medical office setting preferred
  • strong communication and organizational skills
  • ability to multitask and remain calm under pressure
  • proficient with basic office software and technology
  • valid driver's license and use of insured automobile

Job Duties

  • develops and maintains the client care plan, creating extraordinary client experiences and communicating with clients, families, care partners and referral sources
  • plays an integral role in interfacing with clients and families to ensure safe, appropriate and meaningful care is being provided
  • serves as a mentor to caregivers and supports their care delivery
  • maintains a calm, positive demeanor at all times and is able to effectively multitask in a high functioning office environment
  • collaborates with the care team to monitor and adjust care plans as needed
  • coordinates communication between clients, families and care partners
  • participates in training and development to enhance care coordination skills
