Job Requirements
- Certified or willing to be certified as CHR upon request within one year of employment
- certified in basic life support (CPR) or becomes certified within 3 months of employment
- must have a valid driver’s license and driving record that allows access to CPN vehicles for daily work function
- computer skills required for documentation in the patient record
- must have a high school diploma or GED equivalent
- must have a good driving record and personal automobile insurance
- must have very good communication and written skills
- ability to work a varying schedule as needed

Job Duties
- Provide tribal elders safe transportation to and from medical appointments or other destinations as required
- give assistance as needed to get the elder to their appointments including helping with assistive devices or paperwork
- document all mileage, addresses, and client signatures as required on transport logs
- promptly report any problems encountered with the SSN supervisor
- provide friendly and courteous customer service
- perform assigned supportive procedures for the well-being of tribal elders
- provide patient advocacy and case and resource management
- assist with personal care and activities of daily living for patients including nutrition, shopping, running errands, and minor house cleaning
- provide support to CHR nurse as needed
- perform other duties and projects as required by management
- make home visits ordered by physician or SSN Project Director
- participate in community health promotion and disease prevention events
