Job Requirements

  • Certified or willing to be certified as CHR upon request within one year of employment
  • certified in basic life support (CPR) or becomes certified within 3 months of employment
  • must have a valid driver’s license and driving record that allows access to CPN vehicles for daily work function
  • computer skills required for documentation in the patient record
  • must have a high school diploma or GED equivalent
  • must have a good driving record and personal automobile insurance
  • must have very good communication and written skills
  • ability to work a varying schedule as needed

Job Duties

  • Provide tribal elders safe transportation to and from medical appointments or other destinations as required
  • give assistance as needed to get the elder to their appointments including helping with assistive devices or paperwork
  • document all mileage, addresses, and client signatures as required on transport logs
  • promptly report any problems encountered with the SSN supervisor
  • provide friendly and courteous customer service
  • perform assigned supportive procedures for the well-being of tribal elders
  • provide patient advocacy and case and resource management
  • assist with personal care and activities of daily living for patients including nutrition, shopping, running errands, and minor house cleaning
  • provide support to CHR nurse as needed
  • perform other duties and projects as required by management
  • make home visits ordered by physician or SSN Project Director
  • participate in community health promotion and disease prevention events
