Job Requirements
- Applicants must reside in DeKalb County, Illinois or within a 10-mile radius of its border in a neighboring county
- one or more years of experience in health care and/or case management
- one or more years of experience with Medicare and Medicaid recipients, long-term care services, Home and Community-Based Services, and/or managed care organizations
- intermediate to advanced computer skills and experience with Microsoft Word, Excel, and Outlook
- ability to use various electronic information applications including electronic medical records
- exceptional communication and interpersonal skills
- ability to travel regionally for face-to-face meetings
- this role is patient-facing and requires tuberculosis screening
- valid state driver’s license and personal vehicle liability insurance with minimum coverage as per state requirements
Job Qualifications
- Bachelor's degree in social sciences, social work, human services, or related field
- experience in health care or case management
- experience working with Medicare, Medicaid recipients, long-term care services, Home and Community-Based Services, or managed care organizations
- intermediate to advanced computer skills including Microsoft Word, Excel, and Outlook
- ability to use electronic medical records software
- exceptional communication and interpersonal skills
- ability to travel locally to meet members, families, and care teams
Job Duties
- Visit Medicaid members in their homes, Assisted Living Facilities, and Long-Term Care Facilities
- ensure members receive services in the least restrictive setting to maintain optimal well-being
- identify and resolve barriers hindering effective care
- plan and implement interventions to meet care needs
- coordinate services and evaluate case management plans against member goals
- monitor patient care and progress using assessments and data
- guide members and families toward appropriate healthcare resources
