Job Requirements

  • Applicants must reside in DeKalb County, Illinois or within a 10-mile radius of its border in a neighboring county
  • one or more years of experience in health care and/or case management
  • one or more years of experience with Medicare and Medicaid recipients, long-term care services, Home and Community-Based Services, and/or managed care organizations
  • intermediate to advanced computer skills and experience with Microsoft Word, Excel, and Outlook
  • ability to use various electronic information applications including electronic medical records
  • exceptional communication and interpersonal skills
  • ability to travel regionally for face-to-face meetings
  • this role is patient-facing and requires tuberculosis screening
  • valid state driver’s license and personal vehicle liability insurance with minimum coverage as per state requirements

Job Qualifications

  • Bachelor's degree in social sciences, social work, human services, or related field
  • experience in health care or case management
  • experience working with Medicare, Medicaid recipients, long-term care services, Home and Community-Based Services, or managed care organizations
  • intermediate to advanced computer skills including Microsoft Word, Excel, and Outlook
  • ability to use electronic medical records software
  • exceptional communication and interpersonal skills
  • ability to travel locally to meet members, families, and care teams

Job Duties

  • Visit Medicaid members in their homes, Assisted Living Facilities, and Long-Term Care Facilities
  • ensure members receive services in the least restrictive setting to maintain optimal well-being
  • identify and resolve barriers hindering effective care
  • plan and implement interventions to meet care needs
  • coordinate services and evaluate case management plans against member goals
  • monitor patient care and progress using assessments and data
  • guide members and families toward appropriate healthcare resources
