Job Requirements
- High school diploma or equivalent
- Knowledge of community and neighborhoods served by the local WIC program
- Fluency in English
- Availability to work Monday to Friday 8:30 am - 5:00 pm with some nights and weekends
- Ability to maintain an organized work area and collaborate as a team member
- Ability to communicate effectively with patients and community partners
- Maintain confidentiality and professionalism in handling sensitive information
Job Qualifications
- High school diploma or associate's degree preferred
- Knowledge of community and neighborhoods served by the local WIC program
- Fluency in English required
- Bilingual in Portuguese, CV Creole or Spanish preferred
- Minimum of two years experience in community work, outreach, or health and human services preferred
- Excellent oral and written communication skills
- Strong time management skills
- Ability to handle confidential information professionally
- Demonstrated customer service skills and positive approach to patient interaction
Job Duties
- Develop and maintain community liaison with health and human services providers and advocacy organizations
- Build an active, coordinated referral network and collaborative, working relationships between community agencies, groups and WIC
- Promote the utilization of WIC services within the community
- Assist with retention activities to strengthen communication with WIC participants about eligibility and recertification
- Participate in community events, including some nights and weekends as needed
- Maintain accurate records of community outreach and referral activities
- Collaborate with team members to support program goals and improve service delivery
