Job Requirements
- High school diploma or equivalent
- Must be a veteran or have knowledge of military culture preferred
- Ability to work flexible hours including evenings and weekends
- Must complete all sections of the application fully
- Local candidates only
- Ability to maintain confidentiality and professional boundaries
- Valid driver's license and ability to transport clients
- Ability to complete peer certification and other program-specific training as required
- Willingness to participate in case conferences and staff meetings
Job Qualifications
- High school diploma or equivalent
- Experience as a veteran or familiarity with military culture preferred
- Previous experience in peer support, social services, or case management beneficial
- Ability to maintain professional boundaries with clients
- Good communication and interpersonal skills
- Basic computer skills including data entry
- Ability to work flexible hours including evenings and weekends
- Certification in peer specialist or related training preferred
- Undergraduate studies in social sciences or related field preferred for mental health programs
- Knowledge of veteran resources and community agencies
- Ability to facilitate groups and community activities
- Capability to maintain confidentiality according to HIPAA
Job Duties
- Ensures harmonious atmosphere within program by maintaining communication with clinical and program staff and fostering therapeutic community
- Establishes rapport and maintains professional relationships and boundaries with clients and potential clients
- Coordinates individual and group services to help veterans achieve life and recovery goals
- Prevents or de-escalates problematic client behaviors and submits incident reports as needed
- Assists with maintaining compliance with VA, HUD, and other funder regulations by ensuring proper documentation
- Participates in case conferences, consultations, and staff meetings
- Facilitates and participates in monthly Town Hall meetings
- Assists Case Managers with maintaining confidential client files in compliance with organization standards
- Maintains confidentiality according to HIPAA guidelines
- Assists in quality management activities, data collection, and report preparation
- Attends scheduled team meetings and training
- Maintains and submits timesheets, mileage logs, and other documentation accurately and timely
- Addresses client issues timely to ensure uninterrupted services
- Facilitates peer-to-peer groups and life skills classes with supervision
- Assists with monitoring program rent and participant fee compliance
- Identifies and utilizes community resources to meet client needs
- Transports clients to community appointments and provides peer support accompaniment
- Coordinates therapeutic, social, and recreational activities and outings
- Performs other duties as assigned
- Provides peer counseling, referrals, and advocacy to formerly homeless veterans
- Facilitates peer groups and community activities including arts, cooking, and movie nights
- Monitors and engages tenants to respond to issues
- Provides non-clinical support for access to substance use and mental health services
- Ensures timely filing of progress notes
- Assists with client accountability through contact, bed checks, and daily sign-in logs
- Conducts toxicology testing and room inspections to ensure client health and welfare
- Encourages and coordinates client participation in program activities
- Maintains and posts monthly calendars of activities
- Facilitates shuttle van runs to VA and grocery stores
- Acts as liaison with Property Management for housing facilities
- Seeks clinical staff support for crisis intervention
- Complies with Texas Veterans Commission grant requirements as applicable
- Identifies and pre-screens applicants for Rapid Re-Housing eligibility
- Participates in community outreach and VA Stand Downs
- Provides peer support, mentoring, and coaching in housing search and placement
- Assists clients with benefits application process and attendance at related appointments
- Conducts wellness checks, home visits, and HQS inspections
- Collaborates with community agencies and veteran serving organizations
- Completes required certifications and training
- Advocates for veteran healthy functioning through referrals and linkages
- Acts as "Battle Buddy" lead supporting veterans and military family members
- Collaborates with community stakeholders and funders
- Maintains accurate client records and reports outcomes
- Coordinates with Income & Benefits Specialist on client assistance
