Job Requirements
- Current, unencumbered RN license in the state of employment
- Minimum of 1 year of RN experience in relevant healthcare settings
- Current CPR certification
- Valid driver’s license, auto insurance, and reliable transportation
- Ability to travel frequently including overnight stays
- Flexibility to work in varied clinical environments
- Commitment to compassionate hospice care
- Compliance with safety-sensitive position requirements
- Adherence to state-specific employment criteria where applicable
Job Qualifications
- Current, unencumbered RN license in the state of employment
- Minimum of 1 year of RN experience (hospice, home health, acute care, long-term care, oncology or other high acuity setting)
- Current CPR certification
- Valid driver’s license, auto insurance, and reliable transportation
- Hospice and end-of-life nursing experience preferred
- Skilled in clinical assessments and compassionate communication preferred
- Strong critical thinking, flexibility, and problem-solving skills preferred
- Comfortable working in varied environments with shifting needs preferred
- Proficient in documentation and electronic health records preferred
- Calm and supportive in crisis situations
- skilled at family engagement preferred
Job Duties
- Deliver high-quality, compassionate nursing care for hospice patients
- Manage and implement individualized plans of care
- Support and lead interdisciplinary team members (LPNs, CNAs, HHAs, etc.)
- Assess and address patients’ physical, emotional, spiritual, and environmental needs
- Accurately document all care provided and ensure compliance with hospice protocols
- Educate families and caregivers on care instructions, medications, and safety
- Participate in the on-call rotation and provide after-hours care as needed
- Collaborate with physicians, referral sources, and the care team
- Assist with referrals, regulatory documentation, and payer compliance
- Travel to various Gentiva hospice locations as assigned to fill urgent needs
