Job Requirements
- Current, unencumbered RN license in the state of employment
- Minimum of 1 year of RN experience
- Current CPR certification
- Valid driver’s license

Job Qualifications
- Current, unencumbered RN license in the state of employment
- Minimum of 1 year of RN experience in hospice, home health, acute care, long-term care, oncology or other high acuity settings
- Current CPR certification
- Valid driver’s license, auto insurance, and reliable transportation
- Skilled in clinical assessments and compassionate communication
- Strong critical thinking, flexibility, and problem-solving skills
- Comfortable working in varied environments with shifting needs
- Proficient in documentation and electronic health records
- Calm and supportive in crisis situations
- Skilled at family engagement
Job Duties
- Deliver high-quality, compassionate nursing care for hospice patients
- Manage and implement individualized plans of care
- Support and lead interdisciplinary team members
- Assess and address patients' physical, emotional, spiritual, and environmental needs
- Accurately document all care provided and ensure compliance with hospice protocols
- Educate families and caregivers on care instructions, medications, and safety
- Participate in on-call rotation and provide after-hours care as needed
- Collaborate with physicians, referral sources, and the care team
- Assist with referrals, regulatory documentation, and payer compliance
- Travel to various hospice locations to fill urgent needs
