Job Requirements

  • Current, unencumbered RN license in the state of employment
  • Minimum of 1 year of RN experience
  • Current CPR certification
  • Valid driver’s license
  • Auto insurance
  • Reliable transportation

Job Qualifications

  • Current, unencumbered RN license in the state of employment
  • Minimum of 1 year of RN experience in hospice, home health, acute care, long-term care, oncology or other high acuity settings
  • Current CPR certification
  • Valid driver’s license, auto insurance, and reliable transportation
  • Skilled in clinical assessments and compassionate communication
  • Strong critical thinking, flexibility, and problem-solving skills
  • Comfortable working in varied environments with shifting needs
  • Proficient in documentation and electronic health records
  • Calm and supportive in crisis situations
  • Skilled at family engagement

Job Duties

  • Deliver high-quality, compassionate nursing care for hospice patients
  • Manage and implement individualized plans of care
  • Support and lead interdisciplinary team members
  • Assess and address patients' physical, emotional, spiritual, and environmental needs
  • Accurately document all care provided and ensure compliance with hospice protocols
  • Educate families and caregivers on care instructions, medications, and safety
  • Participate in on-call rotation and provide after-hours care as needed
  • Collaborate with physicians, referral sources, and the care team
  • Assist with referrals, regulatory documentation, and payer compliance
  • Travel to various hospice locations to fill urgent needs
