Job Requirements
- high school diploma or GED
- minimum 1-year experience in coordinating a program
- experience working in healthcare field or geriatrics setting
- knowledge of electronic health records preferred
- bilingual in English, Spanish, Mandarin, or Cantonese depending on location preferred
- valid BLS certification or higher following American Heart Association or American Red Cross guidelines

Job Duties
- plan, organize, and coordinate daily transportation services for PACE participants
- collaborate with transportation clerk, dispatcher, and drivers to schedule transportation needs
- ensure timely and safe transport of participants
- maintain accurate transportation records and communication logs
- address and resolve transportation issues promptly
- comply with healthcare and safety regulations
- support participant satisfaction through responsive service
