Job Requirements
- Prior experience in skilled nursing facility (SNF), rehab, or healthcare admissions
- Proficiency in relevant software systems like Point Click Care and insurance portals
- Excellent organizational skills with the ability to manage multiple priorities
- Strong verbal and written communication skills with empathy and professionalism
- Ability to collaborate effectively with multidisciplinary teams
- Dependability, positivity, and composure in a fast-paced environment
- High school diploma or equivalent
Job Qualifications
- Experience in skilled nursing facility (SNF), rehabilitation, or healthcare admission processes
- Proficiency in Microsoft Office and Point Click Care (PCC)
- Familiarity with insurance portals including NaviHealth, Availity, UHC
- Experience with hospital referral portals such as AIDA, CarePort, Wellsky, EPIC
- Strong communication and interpersonal skills
- Ability to manage multiple tasks with attention to detail
- Demonstrated ability to work collaboratively with clinical and administrative teams
- High school diploma or equivalent
- Previous customer service or healthcare coordination experience
Job Duties
- Serve as the primary contact for all incoming referrals, calls, walk-ins, and inquiries
- Coordinate admission documentation completion, insurance verification, and pre-admission authorization
- Communicate clearly and professionally with hospitals, case managers, residents, families, and internal departments
- Ensure accurate data entry and timely updates in Point Click Care CRM
- Facilitate facility tours, referral follow-up, and participate conversion strategies
- Maintain strong customer service standards at every touchpoint
- Collaborate with nursing, therapy, and administrative teams to ensure smooth admissions
- Ensure empty rooms are admit and tour ready on a daily basis
- Assist with marketing events, community outreach, and relationship development activities
