Job Requirements

  • Prior experience in skilled nursing facility (SNF), rehab, or healthcare admissions
  • Proficiency in relevant software systems like Point Click Care and insurance portals
  • Excellent organizational skills with the ability to manage multiple priorities
  • Strong verbal and written communication skills with empathy and professionalism
  • Ability to collaborate effectively with multidisciplinary teams
  • Dependability, positivity, and composure in a fast-paced environment
  • High school diploma or equivalent

Job Qualifications

  • Experience in skilled nursing facility (SNF), rehabilitation, or healthcare admission processes
  • Proficiency in Microsoft Office and Point Click Care (PCC)
  • Familiarity with insurance portals including NaviHealth, Availity, UHC
  • Experience with hospital referral portals such as AIDA, CarePort, Wellsky, EPIC
  • Strong communication and interpersonal skills
  • Ability to manage multiple tasks with attention to detail
  • Demonstrated ability to work collaboratively with clinical and administrative teams
  • High school diploma or equivalent
  • Previous customer service or healthcare coordination experience

Job Duties

  • Serve as the primary contact for all incoming referrals, calls, walk-ins, and inquiries
  • Coordinate admission documentation completion, insurance verification, and pre-admission authorization
  • Communicate clearly and professionally with hospitals, case managers, residents, families, and internal departments
  • Ensure accurate data entry and timely updates in Point Click Care CRM
  • Facilitate facility tours, referral follow-up, and participate conversion strategies
  • Maintain strong customer service standards at every touchpoint
  • Collaborate with nursing, therapy, and administrative teams to ensure smooth admissions
  • Ensure empty rooms are admit and tour ready on a daily basis
  • Assist with marketing events, community outreach, and relationship development activities
