Job Requirements
- Basic Cardiac Life Support must be obtained within 7 days of employment start date
- Bachelor's degree in social work
- Must hold LBSW, LMSW/RCSW-I, or LCSW licensure
- Ability to collaborate effectively with multidisciplinary healthcare teams
- Strong communication and assessment skills
- Commitment to patient-centered care and ethical standards
- Flexibility to work PRN/per diem shifts
Job Qualifications
- Basic Cardiac Life Support certification obtained within 7 days of employment start date
- Bachelor’s Degree in Social Work
- Licensed Baccalaureate Social Worker (LBSW), Licensed Master Social Worker or Registered Clinical Social Worker-Intern (LMSW/RCSW-I), or Licensed Clinical Social Worker (LCSW)
Job Duties
- Work collaboratively with RN case managers, acute care nurses, physicians, insurance companies, and other staff members to proactively identify and address patient needs
- Identify and address barriers to discharge and recovery including financial, emotional, psychosocial, medical, and spiritual factors
- Perform thorough assessments and develop appropriate discharge plans for high-risk patients with discharge planning or psychosocial needs
- Obtain input from the RN case manager to ensure the discharge plan is appropriate and aligns with the patient care plan
- Reassess and modify patient discharge plans in collaboration with the clinical team as needed
- Participate in interdisciplinary care planning and perform proactive discharge planning
- Provide education to patients and caregivers regarding community resources, post-acute care criteria, benefits, and coverage
- Coordinate and facilitate meetings between the care team and patients, families, or caregivers
- Assist patients and families in making difficult decisions while honoring their preferences and values
