Job Requirements
- current unrestricted RN licensure in state of practice
- current driver's license
- vehicle insurance
- access to a dependable vehicle or public transportation
- ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client
Job Qualifications
- current unrestricted RN licensure in state of practice
- ability to function in any home situation regardless of patient demographics
- home care experience preferred
- current CPR certification preferred
- able to work independently
- good communication, writing, and organizational skills
Job Duties
- provides clinical services within the scope of nursing as per state laws and plan of care
- receives referrals and ensures appropriate clinician assignments
- evaluates patient orders and schedules start of care visits
- coordinates determination of patient home health benefits and insurance approvals
- continuously assesses patient needs and ensures care is individualized, appropriate, and meets eligibility criteria
- reviews assessments and plans of care daily and consults clinicians with recommendations
