Job Requirements
- PharmD from an ACPE-accredited institution required
- Current California Registered Pharmacist license required
- Current BLS/Healthcare Provider status per American Heart Association required
- Two years experience in clinical practice setting required
- Experience in a Specialty Pharmacy setting required
- Ability to operate computer and working knowledge of word processing, spreadsheets, and presentations
- Proficient with electronic medical records and clinical documentation
- Strong interpersonal skills
- Effective verbal, written, and listening skills
- Able to work collaboratively and take direction
- Positive telephone manners
- Certified Specialty Pharmacist (CSP) credential preferred
- Completion of an ASHP-accredited PGY-1 Pharmacy Residency or comparable work experience preferred

Job Duties
- Ensure that the patient's pharmaceutical needs are effectively met by accurately verifying prescription orders and dispensing pharmaceuticals in a timely manner
- Review and confirm that prescriptions are written for a legitimate reason under the usual scope of practice of the prescriber
- Ensure the appropriateness of drug, dose, and frequency for each medication ordered before it is dispensed
- Clarify any ambiguities with the prescriber
- Ensure proper storage, handling, and documentation of specialty medications
- Complete critical patient monitoring and review patient profile/chart to identify, prevent, or mitigate drug related problems
- Oversee Specialty Pharmacy operations and technician practice/activities
- Provide patient education and assist with coordination of care involving family members, caregivers, social services, and other healthcare providers
- Serve as a resource for drug information including indications, doses, routes, side effects, adverse reactions, drug interactions, pharmacokinetics, and product availability
- Facilitate specialty medication procurement, ordering, and dispensing procedures
- Exercise professional judgment in providing therapeutic and drug information to patients and other health professionals
- Make appropriate evidence-based, patient-centered medication recommendations
- Document clear clinical progress notes to support patient safety oversight
- Work as an active member of multidisciplinary team and collaborate with healthcare providers
- Initiate prior authorization process and coordinate additional clinical data provision to insurers
- Manage prior authorization, insurance approvals, and financial patient assistance programs
- Assist in preparation for Specialty Pharmacy accreditation and maintain accreditation standards
- Provide in-depth counseling and education to patients on medication use, administration techniques, potential side effects, and adherence strategies
- Recommend therapeutically equivalent medications to prescribers when appropriate
- Participate in the discharge process, streamlining services related to discharge prescriptions
- Promote a positive working environment and strong employee morale
- Maintain accuracy and confidentiality of patient Electronic Health Record records
- Ensure Specialty Pharmacy's role supports the delivery of highest standard patient care and customer service
- Ensure compliance with REMS programs
- Participate in quality assurance, regulatory compliance and safety initiatives
- Perform other duties as assigned
