Job Requirements

  • Valid Class C Texas driver’s license or equivalent
  • Ability to travel up to 60 percent
  • Minimum of 2 years experience in case management preferred
  • Strong organizational skills with ability to meet deadlines
  • Experience with computer software, databases, and reporting tools
  • Ability to maintain confidentiality
  • Ability to work under minimal supervision
  • Excellent oral and written communication skills
  • Must comply with DSHS policies and standards of conduct
  • Ability to provide own transportation
  • Current professional license to practice social work or registered nurse license in Texas

Job Qualifications

  • Current professional license to practice social work in Texas such as LBSW, LMSW, or LCSW
  • Or current registered nurse license to practice in Texas
  • Knowledge of case management principles and children's health programs
  • Skill in conducting family needs assessments and developing service plans
  • Ability to work independently with minimal supervision
  • Strong communication and presentation skills
  • Ability to work with interdisciplinary teams
  • Experience with computer software including MS Office suite

Job Duties

  • Perform case management for children and pregnant women programs
  • Conduct comprehensive family needs assessments and develop individual service plans
  • Link families to community resources and conduct home visits
  • Maintain accurate records and enter data in specified reporting systems
  • Participate in community outreach and provide presentations
  • Ensure adherence to quality assurance policies and complete corrective actions
  • Assist with disaster response and other duties as assigned
