Job Requirements
- Valid Class C Texas driver’s license or equivalent
- Ability to travel up to 60 percent
- Minimum of 2 years experience in case management preferred
- Strong organizational skills with ability to meet deadlines
- Experience with computer software, databases, and reporting tools
- Ability to maintain confidentiality
- Ability to work under minimal supervision
- Excellent oral and written communication skills
- Must comply with DSHS policies and standards of conduct
- Ability to provide own transportation
- Current professional license to practice social work or registered nurse license in Texas
Job Qualifications
- Current professional license to practice social work in Texas such as LBSW, LMSW, or LCSW
- Or current registered nurse license to practice in Texas
- Knowledge of case management principles and children's health programs
- Skill in conducting family needs assessments and developing service plans
- Ability to work independently with minimal supervision
- Strong communication and presentation skills
- Ability to work with interdisciplinary teams
- Experience with computer software including MS Office suite
Job Duties
- Perform case management for children and pregnant women programs
- Conduct comprehensive family needs assessments and develop individual service plans
- Link families to community resources and conduct home visits
- Maintain accurate records and enter data in specified reporting systems
- Participate in community outreach and provide presentations
- Ensure adherence to quality assurance policies and complete corrective actions
- Assist with disaster response and other duties as assigned
