Job Requirements

  • Master's degree in Social Work
  • One year of relevant work or clinical experience for Social Worker I
  • Three years of relevant work experience for Social Worker II
  • Previous experience in clinic, hospital, or community health setting
  • Ability to analyze and interpret industry-related documents and reports
  • Ability to write reports and present information effectively
  • Basic computer knowledge including Word, Excel, Outlook
  • Experience with Electronic Health Record (EHR) systems
  • Valid Washington State Driver's License with acceptable driving record
  • Proof of COVID-19 vaccination required excluding Johnson & Johnson vaccine unless supplemented
  • Proof of immunizations and/or immunity to MMR, Varicella, influenza, TB, Tdap, and Hepatitis B as applicable prior to employment.

Job Qualifications

  • Master's degree in Social Work
  • Relevant clinical experience in a healthcare setting
  • Experience working with vulnerable populations
  • Familiarity with behavioral health and community health
  • Proficiency with electronic health records and telehealth platforms
  • Ability to conduct biopsychosocial assessments
  • Strong communication and interpersonal skills
  • Experience in crisis intervention and care coordination
  • Ability to work collaboratively in interdisciplinary teams
  • Bilingual proficiency preferred
  • Valid Washington State driver's license with acceptable driving record.

Job Duties

  • Provide advanced biopsychosocial evaluation, assessment, triage, referral, and support to patients with complex medical, behavioral health and social needs by actively partnering with interdisciplinary care teams
  • Develop, maintain, and advance individualized care plans with patients focusing on high-risk individuals and health equity
  • Provide intake, longitudinal care management, and social work support including behavioral health treatment planning, crisis intervention, and resource navigation
  • Act as team and organizational resource providing social work expertise and assist with training and mentoring of peers
  • Maintain engaged patient panel using person-centered techniques like motivational interviewing, health coaching, and critical thinking
  • Provide advanced psychosocial support in situations involving domestic violence, homelessness, trauma, substance use, and complex social or behavioral health issues
  • Collaborate with external community partners and organizations to promote effective care delivery and patient wellness
  • Administer and interpret biopsychosocial screenings and assessments and utilize data to inform care plans
  • Complete payer contract requirements and provide services aligned with programmatic and managed care organization standards
  • Advocate for patient self-management, person-centered care and health equity initiatives within the care team
  • Maintain confidentiality, professionalism, and appropriate patient boundaries while supporting patient safety and mandated reporting
  • Document appropriately and timely in electronic medical record systems and participate in staff meetings and trainings.
