Job Requirements

  • Bachelor’s degree in social work
  • One or more years experience in social work
  • Ability to work and communicate with people of diverse social, economic, and cultural backgrounds
  • Strong interpersonal communication and negotiation skills
  • Critical thinking and problem-solving skills
  • Customer service skills
  • Effective organizational skills
  • Computer proficiency with Outlook and electronic medical records
  • Flexibility and adaptability in a complex healthcare environment
  • Understanding of pre-acute and post-acute venues of care
  • Knowledge of local community resources especially for patients with limited payment resources
  • Psychosocial assessment and intervention abilities
  • Capability to provide grief counseling, crisis intervention and de-escalation services
  • Ability to conduct motivational interviewing for patients with substance use disorders
  • Experience in patient and family education and advocacy
  • Familiarity with hospital and community agency collaboration
  • Preferred care management experience
  • Preferred knowledge of state and federal care management guidelines

Job Duties

  • Provide patient and family advocacy
  • Support patient’s choice and patient rights during hospitalization
  • Communicate with payors for authorization for post-acute care
  • Document discharge planning evaluation, ongoing assessment, discharge plans, multidisciplinary rounds, barriers to progression of care, avoidable days, and patient and family needs
  • Assess patients’ and families’ needs holistically for discharge planning including prior functioning, support systems, financial, and psychosocial factors
  • Review medical records including medications, history and physical, labs, and progress notes
  • Develop discharge plans with contingency plans to ensure timely care coordination
  • Communicate with post-acute care services and facilities to ensure continuity of care
  • Participate in multi-disciplinary rounds to review patient status and discharge plans
  • Educate patients and families about emotional, social, and financial impacts of illness
  • Mobilize family and community resources
  • Assess readmitted patients for causes of readmission
  • Facilitate patient and family care conferences with the multidisciplinary team
