Job Requirements

  • High school diploma or equivalent
  • experience in social services or healthcare setting preferred
  • excellent communication skills

Job Qualifications

  • Bachelor's or associate's degree in social work or a related field preferred
  • healthcare or long-term care experience highly preferred
  • ability to meet any applicable state and/or federal requirements
  • basic understanding of computer technology and office systems
  • strong organizational, communication, and interpersonal skills
  • ability to read, write, and follow oral and written instructions in English
  • ability to relate positively to residents and families and work cooperatively with team members

Job Duties

  • Assist with developing comprehensive social history and psychosocial assessments for new admissions
  • assist with discharge planning, care plans, and completion of MDS documentation as assigned
  • orient new residents and support their adjustment to the facility
  • assist residents and families with transitions, including room changes
  • support families with engraving and/or marking residents' personal belongings
  • review residents' rights with residents and staff and assist with personal resident needs
  • assist in facilitating family meetings and supporting volunteer programs
  • document progress notes related to residents' care plans in accordance with policy
  • greet and assist residents, families, visitors, and vendors in a professional and courteous manner
  • answer and route incoming phone calls accurately and promptly
  • maintain logs, schedules, and records as required
  • provide clerical and administrative support to nursing and interdisciplinary teams
  • assist with filing, copying, data entry, and maintaining confidential records
  • support communication between departments to ensure smooth daily operations
