Job Qualifications
- Bachelor's or associate's degree in social work or a related field preferred
- healthcare or long-term care experience highly preferred
- ability to meet any applicable state and/or federal requirements
- basic understanding of computer technology and office systems
- strong organizational, communication, and interpersonal skills
- ability to read, write, and follow oral and written instructions in English
- ability to relate positively to residents and families and work cooperatively with team members
Job Duties
- Assist with developing comprehensive social history and psychosocial assessments for new admissions
- assist with discharge planning, care plans, and completion of MDS documentation as assigned
- orient new residents and support their adjustment to the facility
- assist residents and families with transitions, including room changes
- support families with engraving and/or marking residents' personal belongings
- review residents' rights with residents and staff and assist with personal resident needs
- assist in facilitating family meetings and supporting volunteer programs
- document progress notes related to residents' care plans in accordance with policy
- greet and assist residents, families, visitors, and vendors in a professional and courteous manner
- answer and route incoming phone calls accurately and promptly
- maintain logs, schedules, and records as required
- provide clerical and administrative support to nursing and interdisciplinary teams
- assist with filing, copying, data entry, and maintaining confidential records
- support communication between departments to ensure smooth daily operations
