Job Requirements
- Valid nursing license for RNs or LPNs
- Certification as CNA or HHA where applicable
- Minimum of one year experience in patient care or relevant healthcare setting
- Ability to pass background checks and drug screening
- Commitment to providing patient-centered hospice care
- Must meet all state-specific healthcare requirements
- Willingness to participate in ongoing education and training
Job Qualifications
- Licensed Registered Nurse (RN) in the state of employment
- Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) credential
- Certified Nursing Assistant (CNA) or Home Health Aide (HHA) certification
- Strong interpersonal and communication skills
- Compassionate approach to patient care
- Ability to work independently and as part of a team
- Experience in hospice, palliative, or home health care preferred
Job Duties
- Conduct patient assessments and monitor care needs
- Administer medications and treatments according to care plans
- Collaborate with interdisciplinary teams to develop and update patient care plans
- Provide emotional and spiritual support to patients and families
- Document patient progress and communications accurately
- Educate patients and families about disease processes and care options
- Participate in hospice team meetings and training sessions
