Job Requirements

  • Valid nursing license for RNs or LPNs
  • Certification as CNA or HHA where applicable
  • Minimum of one year experience in patient care or relevant healthcare setting
  • Ability to pass background checks and drug screening
  • Commitment to providing patient-centered hospice care
  • Must meet all state-specific healthcare requirements
  • Willingness to participate in ongoing education and training

Job Qualifications

  • Licensed Registered Nurse (RN) in the state of employment
  • Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) credential
  • Certified Nursing Assistant (CNA) or Home Health Aide (HHA) certification
  • Strong interpersonal and communication skills
  • Compassionate approach to patient care
  • Ability to work independently and as part of a team
  • Experience in hospice, palliative, or home health care preferred

Job Duties

  • Conduct patient assessments and monitor care needs
  • Administer medications and treatments according to care plans
  • Collaborate with interdisciplinary teams to develop and update patient care plans
  • Provide emotional and spiritual support to patients and families
  • Document patient progress and communications accurately
  • Educate patients and families about disease processes and care options
  • Participate in hospice team meetings and training sessions
