Job Requirements
- Valid RN license in the state of employment
- Certification as LPN or LVN when applicable
- CNA or HHA certification
- Minimum high school diploma or equivalent
- Ability to pass background check and drug screening
- Availability to work flexible hours including weekends
- Commitment to provide compassionate care and maintain patient confidentiality
Job Qualifications
- Registered Nurse (RN) license in the state of employment
- Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) certification
- Certified Nursing Assistant (CNA) or Home Health Aide (HHA) certification
- Strong interpersonal and communication skills
- Commitment to patient-centered and compassionate care
- Ability to work well within a multidisciplinary team
- Previous hospice or palliative care experience preferred
Job Duties
- Conduct patient assessments and coordinate care plans
- Administer medications and treatments as prescribed
- Provide compassionate end-of-life care and support
- Educate patients and families on hospice services and care options
- Collaborate with interdisciplinary care teams to ensure patient needs are met
- Maintain accurate patient records and documentation
- Participate in ongoing training and professional development
