Job Duties
- Provide patient-focused care in hospice settings
- Conduct comprehensive patient assessments and develop care plans
- Manage and alleviate symptoms of patients with serious or terminal illnesses
- Collaborate with interdisciplinary teams to ensure coordinated care delivery
- Build strong relationships with patients, families, and referral sources
- Participate in on-site interviews and career development activities
- Attend and contribute to team meetings and educational programs
