Job Requirements

  • Current RN license to practice in state of employment
  • CPR certification
  • Valid driver’s license and automobile insurance
  • Minimum of three years hospice experience
  • Ability to work full-time
  • Compassionate and sensitive to terminally ill patients’ needs
  • Professional maturity and tact
  • Flexibility to manage shifting priorities
  • Commitment to patient advocacy

Job Qualifications

  • Licensed nurse with three to five years of hospice experience
  • BSN with five or more years of hospice experience preferred
  • Prior experience as discharge planner, case manager, or educator preferred
  • Strong communication and interpersonal skills
  • Excellent patient assessment and documentation skills
  • Ability to supervise staff and coordinate services
  • Familiarity with community resources and referral networks

Job Duties

  • Receive and respond to referrals from healthcare professionals
  • Assess patient eligibility for hospice services through medical record review and clinical evaluation
  • Educate patients and families on hospice care details
  • Complete documentation and coordinate intake and admission processes
  • Serve as liaison between referral sources and hospice care team
  • Make daily visits to hospitals and healthcare facilities
  • Build and maintain relationships with referral partners and identify outreach opportunities
