Job Requirements
- Current RN license to practice in state of employment
- CPR certification
- Valid driver’s license and automobile insurance
- Minimum of three years hospice experience
- Ability to work full-time
- Compassionate and sensitive to terminally ill patients’ needs
- Professional maturity and tact
- Flexibility to manage shifting priorities
- Commitment to patient advocacy
Job Qualifications
- Licensed nurse with three to five years of hospice experience
- BSN with five or more years of hospice experience preferred
- Prior experience as discharge planner, case manager, or educator preferred
- Strong communication and interpersonal skills
- Excellent patient assessment and documentation skills
- Ability to supervise staff and coordinate services
- Familiarity with community resources and referral networks
Job Duties
- Receive and respond to referrals from healthcare professionals
- Assess patient eligibility for hospice services through medical record review and clinical evaluation
- Educate patients and families on hospice care details
- Complete documentation and coordinate intake and admission processes
- Serve as liaison between referral sources and hospice care team
- Make daily visits to hospitals and healthcare facilities
- Build and maintain relationships with referral partners and identify outreach opportunities
