Job Requirements
- Diploma ADN and or BSN from an accredited institution
- Current Maryland State license as a Registered Nurse
- Two years of nursing experience or equivalent combination of education and experience
- Knowledge of universal precautions for infection control and safe nursing standards
- Strong time management skills
- Excellent interpersonal and communication skills
- Ability to be patient and family focused
- Skill in use of electronic medical record systems or ability to learn

Job Duties
- Complete initial assessment of patient and family to ascertain home care needs including physical emotional spiritual and mental status
- Prepare and implement the care plan for patient and family unit in collaboration with the interdisciplinary group
- Provide direct nursing care to patients to maintain highest level of care in conjunction with the primary physician
- Plan and supervise delivery of care by home health aide
- Cooperate with clinical manager in utilization of other team members outside agencies and community resources
- Update primary physician and interdisciplinary team regularly including patient care conferences
- Teach family members facility staff and hired caregivers patient care skills such as pain control medication administration injections blood draws IV management dressing changes hygiene nutrition and vital signs according to physician orders
- Ensure arrangements for equipment necessary in the facility setting are made
- Attend to patient family and facility at time of patient’s death
- Document in facility chart and maintain positive relationships and communication with facility staff
- Reconcile medications against facility medication list
- Attend care plan meetings and patient care rounds within facility
- Establish and maintain professional relationships with various facility staff including ED DON medicine techs aides nurses rehab staff housekeeping
- Provide educational in-services to facility staff formally and informally
- Participate in marketing and customer service activities with business development managers
