Job Requirements

  • Valid Arkansas Registered Nursing License
  • Minimum of three years RN experience in a clinical environment
  • Current BLS certification
  • Ability to work collaboratively with interdisciplinary teams
  • Proof of legal authority to work in the United States
  • Willingness to undergo pre-employment screenings including criminal background check, drug testing, and education verification
  • Ability to perform physical activities such as sitting, standing, walking, grasping, and manual dexterity
  • Excellent communication skills
  • Commitment to patient-centered care principles

Job Qualifications

  • Baccalaureate Degree, ADN, or Diploma in Registered Nurse
  • Valid Arkansas Registered Nursing License
  • Minimum of three years RN experience in a clinical environment with current BLS certification
  • BSN preferred
  • Care management experience in an Ambulatory setting preferred
  • Experience in continuous quality improvement preferred
  • Knowledge or experience in Patient-Centered Medical Home (PCMH) desired
  • Certification in Case Management (CCM, ANCC) or obtain within one year of hire date preferred
  • Strong clinical knowledge and experience in managing chronic conditions
  • Excellent communication and interpersonal skills
  • Proficiency in care coordination and transitional care management
  • Ability to develop and implement patient-centered care plans
  • Knowledge of value-based care principles and program measures
  • Organizational and problem-solving skills
  • Commitment to promoting patient wellness and self-management

Job Duties

  • Triage, assess, plan, implement, and evaluate nursing care and patient education via phone or in-clinic using the care management model
  • Develop and manage care plans, assess progress, address barriers, and coordinate care for patients across inpatient and outpatient settings
  • Deliver nursing care aligned with UAMS, ANA, AR State Board of Nursing, and specialty organization standards
  • Evaluate patient progress toward desired outcomes and ensure care meets regulatory and clinical standards
  • Coordinate patient education in collaboration with physician staff
  • Initiate performance improvement and evidence-based practice/research processes
  • Collaborate effectively with nursing staff, interdisciplinary teams, leaders, and stakeholders
