Job Requirements
- Valid Arkansas Registered Nursing License
- Minimum of three years RN experience in a clinical environment
- Current BLS certification
- Ability to work collaboratively with interdisciplinary teams
- Proof of legal authority to work in the United States
- Willingness to undergo pre-employment screenings including criminal background check, drug testing, and education verification
- Ability to perform physical activities such as sitting, standing, walking, grasping, and manual dexterity
- Excellent communication skills
- Commitment to patient-centered care principles
Job Qualifications
- Baccalaureate Degree, ADN, or Diploma in Registered Nurse
- Valid Arkansas Registered Nursing License
- Minimum of three years RN experience in a clinical environment with current BLS certification
- BSN preferred
- Care management experience in an Ambulatory setting preferred
- Experience in continuous quality improvement preferred
- Knowledge or experience in Patient-Centered Medical Home (PCMH) desired
- Certification in Case Management (CCM, ANCC) or obtain within one year of hire date preferred
- Strong clinical knowledge and experience in managing chronic conditions
- Excellent communication and interpersonal skills
- Proficiency in care coordination and transitional care management
- Ability to develop and implement patient-centered care plans
- Knowledge of value-based care principles and program measures
- Organizational and problem-solving skills
- Commitment to promoting patient wellness and self-management
Job Duties
- Triage, assess, plan, implement, and evaluate nursing care and patient education via phone or in-clinic using the care management model
- Develop and manage care plans, assess progress, address barriers, and coordinate care for patients across inpatient and outpatient settings
- Deliver nursing care aligned with UAMS, ANA, AR State Board of Nursing, and specialty organization standards
- Evaluate patient progress toward desired outcomes and ensure care meets regulatory and clinical standards
- Coordinate patient education in collaboration with physician staff
- Initiate performance improvement and evidence-based practice/research processes
- Collaborate effectively with nursing staff, interdisciplinary teams, leaders, and stakeholders
