Job Requirements
- Bachelor's degree in social work, social or behavioral sciences, psychology, education, or related fields
- At least one year of experience working with people with developmental disabilities
- Preferred experience with Personal Outcome Measures
- Ability to assist with transferring and lifting up to 50 pounds
- Commitment to treating all people with dignity and respect
- Strong communication and facilitation skills
- Ability to maintain accurate and timely documentation
Job Qualifications
- Bachelor's degree in social work, social or behavioral sciences, psychology, education, or related fields
- At least 1 year of experience working with people with developmental disabilities
- Preferred experience working with Personal Outcome Measures established by The Council
- Ability to assist with physical tasks including transferring and lifting up to 50 pounds
Job Duties
- Promote the growth, well-being, and autonomy of individuals served
- Complete a self-assessment annually with each individual to determine personal outcomes
- Facilitate all plan meetings to discuss strategies for implementing personal outcomes
- Complete documentation requirements including progress on plans, goals, and plan meeting minutes
- Assess data on programs to ensure learning of adaptive and socially productive skills
- Act as a liaison to family members, guardians, team members, and outside entities
- Complete required monthly site visits
- Coordinate and assist individuals in changes of services and environments
- Attend agency staff meetings and participate in in-service training and professional development
