Job Requirements
- Education equivalent to a BA degree in social services, healthcare or related field
- 1-2 years of relevant social work or activities coordination experience
- Ability to pass criminal background check and drug testing
- Willingness to undergo fingerprinting if required
- Regular and predictable attendance
- Capacity to maintain confidentiality and professional behavior
- Must be legally authorized to work in the United States
Job Qualifications
- BA degree in social services, healthcare or related field, or equivalent combination of education and/or experience
- 1-2 years of social service experience working with elderly and families
- Experience coordinating community programs or resident activities
- Strong interpersonal and communication skills
- Ability to develop and maintain supportive relationships with residents
- Familiarity with trauma-informed service delivery
- Proficiency with Microsoft Office and online database systems
- Ability to prepare budgets and program documentation
- Commitment to uphold confidentiality and resident privacy
- Flexibility to work varied schedules as needed
Job Duties
- Coordinate and oversee delivery of services and program activities to residents both on and off-site
- Provide information, assistance, and resident services referrals and engage in follow-up
- Perform needs assessments to determine beneficial programs and services for residents
- Obtain resident feedback on program effectiveness and quality through annual Resident Satisfaction Survey
- Develop supportive relationships with residents and conduct outreach to encourage participation in resident programs
- Provide and coordinate opportunities for residents to learn new skills, build community, and promote self-sufficiency, including social, educational, and StayWell activities
- Connect residents to food resources and facilitate food distributions as needed
- Establish connections with service providers and maintain community resource directory
- Prepare and maintain monthly budgets, program files, and documents in compliance with regulatory requirements
- Create monthly newsletters and calendars of events and activities
- Maintain collaborative relationships with property managers and REM staff and participate in biweekly meetings
- Participate in company-wide initiatives such as scholarships and digital learning
- Assist in recruiting and coordinating volunteers and vendors
- Maintain resident confidentiality and respectful relationships
- Attend Resident Council meetings and work with council and residents to encourage program participation
- Interface with provider agencies, public agencies, and volunteer groups to maximize program effectiveness
- Monitor agency involvement for quality and compliance
- Attend department and company meetings and training sessions
- Participate in Injury and Illness Prevention Plan
- Maintain regular and predictable attendance
- Perform other duties as assigned
