Job Requirements

  • A minimum of two years of experience in social services, case management, or related fields
  • Proficiency in Microsoft Office (Word, Excel, Outlook, PowerPoint)
  • Strong interpersonal and communication skills with the ability to build trust and maintain confidentiality
  • Must possess a valid driver's license
  • Ability to travel when needed
  • Mobility and flexibility to walk, bend, kneel, lift, and reach as required to perform duties

Job Qualifications

Job Duties

  • Develop a comprehensive understanding of residents through needs assessments and community meetings
  • Survey the resident population to identify priority needs and solicit input on service preferences
  • Develop strategies to link residents with needed services and aid in overcoming access barriers
  • Establish and maintain partnerships with relevant organizations to provide on-site services
  • Implement priority programs and activities through community-based service providers or direct services when necessary
  • Participate in the development and implementation of pilot programs for new service delivery models
  • Collect and accurately record program data and submit timely reports
  • Evaluate program outcomes for quality improvement
  • Report abuse and neglect according to state law
  • Obtain releases and maintain confidentiality appropriately
  • Work with property staff and community agencies to address resident problems proactively
  • Collaborate with site teams to meet property needs and resolve resident conflicts
  • Plan and coordinate recreational, cultural, and social activities
  • Collaborate with the Client Service Team to share information and support fundraising efforts
  • Maintain a comprehensive list of available community resources
  • Attend staff meetings and training sessions
