Job Requirements

  • Bachelor's degree preferred with associates degree and relevant experience considered
  • 5-10 years experience in social services or related field
  • Knowledge of community resources and services
  • Training in aging, elder care, substance abuse and mental health issues
  • Familiarity with eligibility and procedures of federal and state entitlement programs
  • Knowledge of legal issues related to service coordination
  • Ability to work with older adults in a supportive capacity
  • Completion of required training hours as a services coordinator
  • Effective communication and documentation skills
  • Ability to maintain detailed resident files

Job Qualifications

  • Bachelor's degree in social work, gerontology, psychology, or public health preferred
  • Associates degree with relevant experience considered
  • 5-10 years experience in related field
  • Knowledge of community services for the served population
  • Training in aging process, elder services, substance abuse, mental health
  • Awareness of federal and state entitlement program eligibility and procedures
  • Knowledge of legal liability in service coordination
  • Experience working directly with older adults
  • Completion of 36 related classroom or seminar training hours before or within first 12 months of hire

Job Duties

  • Work as a team member with community administrator in serving project residents including following up on resident situations that may need supportive services
  • Provide or coordinate educational sessions for residents about available community resources
  • Promote wellness activities for all residents
  • Educate residents on service availability, application procedures, and resident rights
  • Provide intake, assessment, and referral services
  • Monitor the delivery of services to ensure they are appropriate, timely and satisfactory
  • Assist and advise residents and families on services needed to maintain a self-reliant lifestyle
