Job Requirements
- Bachelor's degree preferred with associates degree and relevant experience considered
- 5-10 years experience in social services or related field
- Knowledge of community resources and services
- Training in aging, elder care, substance abuse and mental health issues
- Familiarity with eligibility and procedures of federal and state entitlement programs
- Knowledge of legal issues related to service coordination
- Ability to work with older adults in a supportive capacity
- Completion of required training hours as a services coordinator
- Effective communication and documentation skills
- Ability to maintain detailed resident files
Job Qualifications
- Bachelor's degree in social work, gerontology, psychology, or public health preferred
- Associates degree with relevant experience considered
- 5-10 years experience in related field
- Knowledge of community services for the served population
- Training in aging process, elder services, substance abuse, mental health
- Awareness of federal and state entitlement program eligibility and procedures
- Knowledge of legal liability in service coordination
- Experience working directly with older adults
- Completion of 36 related classroom or seminar training hours before or within first 12 months of hire
Job Duties
- Work as a team member with community administrator in serving project residents including following up on resident situations that may need supportive services
- Provide or coordinate educational sessions for residents about available community resources
- Promote wellness activities for all residents
- Educate residents on service availability, application procedures, and resident rights
- Provide intake, assessment, and referral services
- Monitor the delivery of services to ensure they are appropriate, timely and satisfactory
- Assist and advise residents and families on services needed to maintain a self-reliant lifestyle
