Job Requirements

  • Minimum of a bachelor's degree in gerontology, social work or related field
  • At least three years of experience in social services focused on elderly populations
  • Knowledge of relevant legal liability issues
  • Commitment to serving elderly populations with physical, functional, social and cognitive challenges
  • Strong written and verbal communication skills
  • Excellent organizational skills
  • Proficiency with MS Office and Excel spreadsheets

Job Qualifications

  • Bachelor's degree in gerontology, social work or related field
  • Minimum three years of experience providing social services to elderly populations
  • Knowledge of legal liability issues relevant to social services for at-risk populations
  • Strong written and verbal communication skills
  • Ability to problem-solve, educate and advocate for vulnerable adults
  • Excellent organizational skills
  • Proficiency in MS Office and experience with Excel spreadsheets
  • Master's degree or AASC Professional Services Coordinator certification preferred but not required

Job Duties

  • Develop relationships and partnerships with local agencies and nonprofits to provide services for residents
  • Link and develop ongoing educational programming, wellness presentations, and onsite events that promote resident self-sufficiency
  • Demonstrate working knowledge of federal, state, and local resources including programs available to seniors
  • Serve as liaison with medical and professional service providers and community agencies as authorized
  • Report abuse, neglect, or exploitation of residents in compliance with state statutes and ethical standards
  • Provide limited case management when requested and authorized by the resident
  • Maintain resident files in compliance with HUD and company requirements and prepare regulatory reports
  • Develop and maintain a resource directory for the apartment community
  • Assist residents, families, and representatives with transitions into and out of the facility
  • Accompany Community Director on apartment inspections and coordinate plans for residents to age in place
  • Create monthly calendar and site newsletter with input from staff
  • Attend home visits of prospective tenants to explain Service Coordination program and assist in move-in process
