Job Requirements
- Must have a minimum ASN degree from accredited college or university
- Must be currently licensed or registered in applicable State
- Must maintain an active Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in good standing throughout employment
- Must have two years supervisory experience in senior care or related setting
- Must have CPR certification upon hire or obtain during orientation, certification must remain current during employment
- Must have current First Aid certification if required by State
- Must be able to communicate effectively with physicians, resident care staff, and all other staff, family members, or others
- Must possess the ability to make independent decisions when circumstances warrant such action
- Must be knowledgeable of nursing administration practices and procedures as well as the laws, regulations, and guidelines governing nursing functions in an assisted living community
- Must perform proficiently in all applicable competency areas
- Must maintain confidentiality of all proprietary and/or confidential information
- Must display integrity and professionalism by adhering to Century Park's Code of Ethics and completing all required compliance training
- Must understand and follow company policies including harassment and compliance procedures
Job Qualifications

- Ability to develop and implement care plans and quality improvement initiatives
- Competency in data collection and analysis
- Demonstrated ability to manage medication programs and audits
- Professionalism and integrity in line with company code of ethics
- Ability to work collaboratively with interdisciplinary teams
- Experience in incident investigation and resolution
- Aptitude for time management and organization
Job Duties
- Plan, develop, organize, implement, and evaluate resident care goals
- Maintain compliance with budget in labor and expense control
- Create, manage and approve scheduling for Resident Care staff to ensure appropriate staffing levels 24/7
- Build relationships with residents and families in collaboration with Executive Director or Assisted Living Director
- Complete resident assessments, develop and implement service plans, and communicate outcomes with residents and/or families
- Conduct incident report follow-up investigations and ensure completion of resident status documentation
- Prepare for survey readiness and ensure timely Plan of Correction completion
- Develop and monitor quality improvement plans
- Complete audits to ensure compliance with infection control and other regulations
- Ensure documentation of resident clinical status and communicate with physicians and healthcare professionals
- Investigate and resolve resident and family concerns in collaboration with Executive Director
- Oversee medication program including accuracy of medication records and hands-on assistance
- Communicate and follow up with provider pharmacy for compliance
- Conduct regular audits of medication records and storage
- Recruit, hire, train, evaluate, supervise, and counsel Resident Care team
- Serve as an effective role model with an approachable leadership style
- Communicate effectively with physicians, staff, families, and others
- Participate in community health matters
- Liaison with residents, families, and support departments for care planning
- Collect, analyze, and manage data
- Make independent decisions as needed
- Maintain confidentiality and adhere to company policies
- Participate in compliance and ethics training
- Maintain professional working relationships across departments
