Job Qualifications

  • Active LVN or LPN license
  • Experience in nursing or senior care environment preferred
  • Strong communication skills
  • Ability to supervise and lead a care team
  • Knowledge of electronic medical records systems
  • Excellent organizational skills
  • Commitment to resident-centered care

Job Duties

  • Review 24-hour log
  • Receipt and record review for new move-in
  • EMR dashboard review and update
  • Census review and update
  • Review of incidents and regulatory required reporting
  • Pharmacy order updates in eMAR
  • Document uploading
  • Complete resident assessments and service plans
  • Attend and participate in meetings
  • Provide supervision and guidance to care associates
  • Participate in on-call rotation for after hours or weekend care-related concerns
