Job Requirements
- One year or more of certified home health experience as an RN
- Valid driver’s license
- Reliable transportation
- Car insurance coverage
- Current CPR certification
- Ability to work independently and manage time effectively
Job Qualifications
- Current and unencumbered RN license
- Ability to perform comprehensive patient assessments
- Proficiency in wound care and wound vac
- Experience with HomeCare HomeBase EMR preferred
- Strong communication and interpersonal skills
- Current CPR certification
- Additional certifications in home health or geriatric care preferred
Job Duties
- Provide independent, exceptional care to patients in their homes including comprehensive assessments and creating individualized care plans
- Document care in a timely and thorough manner using HomeCare HomeBase EMR
- Perform wound care and wound vac management
- Coordinate with healthcare teams to ensure continuity of care
- Maintain patient confidentiality and adhere to HIPAA guidelines
- Educate patients and families on care instructions and health management
- Travel to patient homes reliably with appropriate transportation
