Job Duties
- Coordinate the evaluation process of the defined patient population
- serve as a liaison between referring providers and Vanderbilt clinicians
- develop collaborative patient care plans with the clinical team
- manage pre-admission and post-discharge care transitions
- assist in creating and disseminating patient education materials
- collaborate on discharge and transition planning with multidisciplinary teams
- evaluate care coordination practices and support protocol development based on evidence
