Job Duties

  • Coordinate the evaluation process of the defined patient population
  • serve as a liaison between referring providers and Vanderbilt clinicians
  • develop collaborative patient care plans with the clinical team
  • manage pre-admission and post-discharge care transitions
  • assist in creating and disseminating patient education materials
  • collaborate on discharge and transition planning with multidisciplinary teams
  • evaluate care coordination practices and support protocol development based on evidence
