Job Requirements
- Current RN license in state of requested employment or eligibility to obtain
- One or more years of nursing experience
- Valid driver’s license, automobile insurance, and reliable transportation
- Current CPR certification
- Ability to provide care in patient homes or facilities
- Compassionate, calm, and dependable in urgent care situations
Job Qualifications
- Current RN license or eligibility in state of employment
- One or more years of nursing experience
- Strong clinical judgment and compassionate care approach
- Comfortable working independently and in diverse care settings
- Experience with hospice, home health, oncology, or palliative care preferred
- Proficient in electronic medical records and strong documentation skills
- Current CPR certification
Job Duties
- Provide direct patient care after normal business hours
- Serve as primary liaison between patients, families, physicians, and hospice team during shift
- Respond promptly to urgent calls, patient visits, and symptom management needs
- Evaluate patient and family needs to guide clinical decisions and implement care plans
- Collaborate with interdisciplinary teams including LPNs/LVNs and CNAs
- Accurately document clinical visits and communications
- Participate in quality assessment and performance improvement programs
