Job Requirements

  • Current RN license in state of requested employment or eligibility to obtain
  • One or more years of nursing experience
  • Valid driver’s license, automobile insurance, and reliable transportation
  • Current CPR certification
  • Ability to provide care in patient homes or facilities
  • Compassionate, calm, and dependable in urgent care situations

Job Qualifications

  • Current RN license or eligibility in state of employment
  • One or more years of nursing experience
  • Strong clinical judgment and compassionate care approach
  • Comfortable working independently and in diverse care settings
  • Experience with hospice, home health, oncology, or palliative care preferred
  • Proficient in electronic medical records and strong documentation skills
  • Current CPR certification

Job Duties

  • Provide direct patient care after normal business hours
  • Serve as primary liaison between patients, families, physicians, and hospice team during shift
  • Respond promptly to urgent calls, patient visits, and symptom management needs
  • Evaluate patient and family needs to guide clinical decisions and implement care plans
  • Collaborate with interdisciplinary teams including LPNs/LVNs and CNAs
  • Accurately document clinical visits and communications
  • Participate in quality assessment and performance improvement programs
